Clinical manifestation of salmonellosis

Salmonellosis is caused by the gram-negative bacillus Salmonella enterica, of which there are more than 2,500 serotypes. This disease is transmitted by the faecal-oral route, from animal or human, and with or without intermediary contamination of foodstuffs.

When Salmonella bacteria are ingested, they pass through a person’s stomach and colonize the small and large intestine. There, the bacteria invade the intestinal mucosa and proliferate. The bacteria can invade the lymphoid tissues of the gastrointestinal tract and spread to the bloodstream.

Three clinical forms of salmonellosis:
· Gastroenteritis
· Septicemia
· Enteric fevers

Myalgia and headache are common; however, the cardinal manifestation is diarrhoea. Fever (38°C to 39°C) and chills are also common. At least two-thirds of patients complain of abdominal cramps. The duration of fever and diarrhoea varies, but is usually 2 to 7 days. If the infection spreads to the bloodstream and distant organs, the illness increases in duration and severity and will usually include signs and symptoms related to the organ affected.

Prevention of salmonellosis

Salmonellosis is a disease caused by the bacteria Salmonella. It is usually characterized by acute onset of fever, abdominal pain, diarrhoea, nausea and sometimes vomiting.

Infection of humans with Salmonella results in three main infectious diseases, namely typhoid fever, paratyphoid fever and NTS. Typhoid and paratyphoid fevers are caused by S. Typhi and Salmonella enterica serovar Paratyphi (S. Paratyphi), respectively, and are characterized by gastroenteritis and complications such as septicaemia, immunological symptoms, leukopenia and neurological symptoms.

Prevention requires control measures at all stages of the food chain, from agricultural production, to processing, manufacturing and preparation of foods in both commercial establishments and at home.

The contact between infants/young children and pet animals that may be carrying Salmonella (such as cats, dogs, and turtles) needs careful supervision.

Generally, gram negative bacteria such as Salmonella are susceptible to many disinfectants including 1% sodium hypochlorite, 70% ethanol, 70% propanol, 2% glutaraldehyde, and 4% formaldehyde, as well as phenol, peracetic acid, hydrogen peroxide, quaternary ammonium compounds, and iodophors.

They can also be killed by moist heat (121°C [250°F] for a minimum of 15 min) or dry heat (170°C [338°F] for at least 1 hour). Salmonellae are also susceptible to ozone.

Other strategies for enteric fever prevention include improving sanitation, ensuring the safety of food and water supplies, identification and treatment of chronic carriers of Salmonella serovar Typhi, and the use of typhoid vaccines to reduce the susceptibility of hosts to infection or disease.

Prevention against salmonellosis

To decrease the risk of salmonellosis, both food safety practices and the prevention of transmission from animals are important. Human-to-human transmission, though less common, can also be reduced through preventive measures.

Because Salmonella bacteria are spread through poop, one of the best ways to reduce the risk of illness is to wash hands often with warm water and soap. Make a special effort to wash hands in these situations:

*right after playing with a pet or animal (especially a reptile or chicken)
*right after using the bathroom
*before preparing any food, like cutting up vegetables for dinner
*before eating any food

Any food can become contaminated through cross-contamination, environmental contamination, or by the unwashed hands of food workers. Outbreaks of salmonellosis have been traced to a variety of foods including cantaloupe with rinds that were likely contaminated in the field; alfalfa sprouts grown from contaminated seed; and tomatoes contaminated before or during harvest.

It’s good to remind other members of the family to wash their hands often. People who cook meals should wash their hands before touching any food. It's also a good idea to use water and soap to clean kitchen counters, cutting boards, and knives after they touch raw foods.

Another way to protect against Salmonella infection is to never eat raw or undercooked eggs, meat, chicken, or turkey. Meat, chicken, and turkey should be cooked until they are no longer pink in the center, and eggs should be cooked so they aren't wet and runny. And do not consume unpasteurised milk. Salmonella is found massively in unpasteurized milk, contaminated water, raw or undercooked eggs and meat. Infected livestock contributes to the severity ofthe disease just as much.

Also do not purchase dirty or cracked eggs.

Raw fruit and vegetables make healthy snacks, but be sure to wash them well before starting munching.

Babies and small children without diarrhea who are not toilet trained should wear tight fitting waterproof pants or swimming nappies in swimming pools and changed regularly in the change room. When faecal accidents occur, swimming pools should be properly disinfected.

Food preparers should be especially careful when making food for children, the elderly, or other immunocompromised people since they are more likely to develop severe disease. When caregivers are working with raw poultry or meat, infants should not be fed and diapers should not be changed.

Foods most likely to cause salmonellosis

Salmonellosis infection is caused by a Salmonella bacteria which was first found by Soholerin in 1839, and it isolated by Karl Eberth in 1880 from the mesenteric lymph nodes and spleen of a person died from typhoid fever.

Salmonella transmission to food processing plants and food production equipment is a serious public health issue. Salmonella can enter the food chain at any point: crop, farm, livestock feed, food manufacturing, processing and retailing.

A number of workers handle animals during slaughter and processing, and contamination is possible when Salmonella or any other pathogen is present on the equipment or the workers’ hands or clothing.

Eating contaminated foods, such as raw meat, poultry and eggs that have not been cooked properly, is the most common cause of the illness. As well, raw (unpasteurized) milk and raw milk products, and contaminated water, can also contain the bacteria.

What are the symptoms of salmonellosis?

The most common types of food poisoning is salmonellosis, or Salmonella infection. Salmonella bacteria are part of the natural intestinal flora of many animals.

Good sanitation method to eliminate salmonellosis

Salmonella infection in pets

Pets such as cats and dogs can have salmonellosis and be carriers of the causative organism. Many strains of salmonellae have been isolated from cats and dogs and the incidence may vary from 1 to 36 per cent, depending on the conditions under which the animals live.  Since this is the case, children especially the very young who handle materially contaminated by pets and who are very subject to contracting the disease, may contact salmonellosis from pets.

It may be wondered why animals should have salmonellosis. The reason seems to be that their feeds, especially fish meal, meat meal and bone meal fed to them as supplements, often contain Salmonella bacteria. This is also true for some of the dried types of food used for feeding pets.

Prevention of salmonellosis in dogs and cats can be frustrating because of the tendency of some animals to develop a chronic subclinical carrier state or latent infection.

Nontyphoid salmonellae that infect pets also harbored by many other animals and persist in the environment, making eradication difficult.

Children are very susceptible to infection with Salmonella, and pet can transfer the bacteria to children. Do not let pet lick, especially on the face. Make sure to wash hands properly after playing with pet and make sure pets are properly groomed.  Thorough handwashing plays pivotal part on the prevention of foodborne animal-to-animal and human-to-human salmonellosis.

Salmonellosis is a bacteria disease characterized by sudden onset of headache, abdominal gain, diarrhea, nausea, sometimes vomiting and almost always fever. Food-associated Salmonella infections in the United States are estimated by the US Department of Agriculture to cost $3 billion annually.

Shellfish, egg products, prepared salads, and to some extent, raw and cooked meats have often associated with the transmission of salmonellosis.

Salmonella enteritidis outbreaks are most frequently associated with the consumption of poultry products especially undercooked eggs and chicken.

Raw shellfish may be taken from waters contaminated with Salmonella bacteria and cooked shellfish meats may contaminated with humans since they are usually removed from their shell by hand.

Poultry of all types, beef cattle, and hogs may have salmonellosis or be carriers of the organisms causing the disease; hence under conditions of cooking in which these organisms are not destroyed, they may be transmitted by humans.  

Latest large outbreaks have been linked with eating tomatoes, jalapeno peppers, and peanut butter.

From 1998 through 2003, the US Department of Agriculture-Food Safety Inspection Service reported isolation of Salmonella from 11.2% to 22.5% of broiler and ground chicken samples, respectively.

As it is impossible for many raw foods to be produced free from Salmonella at source, it is important to establish animal husbandry and crop agriculture regimes that can make a positive contribution to minimizing the frequency and level of contamination of these primary raw materials by Salmonella.

Epidemiology of salmonellosis

On a global scale, Salmonella is responsible for an estimated 3 billion human infection annually. The salmonellae are gram-negative, non-spore forming, facultatively anaerobic bacteria in the family Enterobacteriaceae.

Salmonellosis is an illness caused by Salmonella bacteria. An infection with these bacteria usually affects the gastrointestinal system (the stomach and intestines) in humans. In more severe cases, Salmonella can spread to the blood, the bones, or even to the fluid around the brain, but these types of infection are less common.

There are numerous transmission pathways through which humans can be exposed to Salmonella, including a wide range of domestic and wild animals and a variety of foodstuff’s including food of both animal and plant origin.

Infected animals carry Salmonella in their feces and the usual route of infection is through fecal-oral transmission. The epidemiology of Salmonella is, therefore, primarily due to direct or indirect fecal contamination of live animals, food or humans.

Infections caused by the salmonellae are group into three major syndromes: gastroenteritis, typhoid or enteric fever and localized infection outside of the GI tract.

In developed countries, most cases of Salmonella infections are acquired upon ingestion of contaminated poultry and beef products.

Most cases of typhoid fever occur in the developing world, particular the Indian Subcontinent and South East Asia, but the disease is also endemic elsewhere in Asia and in Africa. The disease is also commonly reported from the Middle East.

Salmonellosis

Salmonellosis produces several symptoms. These includes diarrhea that may bloody, stomach cramping that may be severe fever and occasionally, nausea.

Illness caused by a majority of Salmonella serotypes range for mild to severe gastroenteritis, and some in patients, bacteraemia, septicemia and a variety of associated longer-term condition.

The onset symptoms can take anywhere from 6 to 72 hours, but usually occurs around 12 hours after consuming the contaminated food or beverage. In typhoid fever, the incubation period is 7 – 14 days.

Persons with salmonellosis often become carriers of the organism for a period of time after they recovered from the disease. That is, they continue to discharge the organisms in their feces. Because of this, carriers often contaminate their hands with these organisms that may not be removed completely even after thorough washing.

Salmonella species cause illness by means of infection, the organism grows and multiplies in the host’s body and become established on or on the cells or tissue of the host.

Salmonella multiply in the small intestine, colonizing and subsequently invading the intestine issue, producing an enterotoxin and causing an inflammatory reaction and diarrhea.

Salmonella is a type of organism called a bacterium. The salmonella bacteria are rod-shaped called bacillus; they do not form spores, and thus are not especially heat resistant. They are motile (can move about in the water, in foods or other materials in which they are found) and will grow either with or without air (oxygen).

These organisms are very widespread. Like many other kinds of bacteria, Salmonella is able to produce infection when it enters a person’s body.

From the time of probably the first laboratory confirmed outbreak of salmonellosis in 1888, Worldwide, salmonellosis is a leading cause of enteric infectious disease attributable to foods and hundreds of outbreaks of foodborne salmonellosis still occur in most countries every year.

In spite of efforts to improve the general standards of food hygiene, surveillance from many countries indicate an increase in salmonellosis.

Epidemiologic data clearly indicate that on a worldwide basis, the major sources of the Salmonella problem are contamination of raw animal products and mishandling of these products during preparations.

Reduction of the incidence of Salmonella contamination of foods requires number of approaches to the problem, beginning at the farm and going right through to the kitchen.

Foodborne salmonellosis problem can be solved by one of three undertakings:
  • Eradicate salmonellae from feed, farm environments and animals
  • Eliminate salmonellae from the raw foods of animal origin by pasteurization, through cooking or by radicidation (pasteurization by irradiation)
  • Educate people to the dangers of salmonellosis, the sources of salmonellae and the ways to prevent transmission of disease-producing quantities of salmonellae in foods that are prepared in food processing plants, food service establishments, and in home kitchens.

The ordinary symptoms of salmonellosis are abdominal pain, diarrhea, chills, frequent vomiting and prostration. However, there are instances in which much more severe symptoms may be encountered. The incubation period (time after ingesting the organisms until symptoms are evident) is 7 – 72 hr. In typhoid fever, the incubation period is 7 – 14 days.

Hence, if carriers handle foods that after to be eaten by others they may contaminate them with these bacteria, and in this manner, transmit the disease to others.

In most cases the carrier stage does not persist longer than 12 weeks after symptoms with salmonellosis and for shorter periods with typhoid fever. However, there are isolated cases in which the carrier stage lasts much longer than 12 weeks and 2 -5 % of those ill with typhoid fever may become permanent carriers.

At the present time, more than 2,000 types of Salmonella bacteria are known, all of which are considered to be ineffective to man. Obviously, these organisms are very widespread.

Whereas it is considered that many Salmonella bacteria must be taken in to cause the disease in a normal adult, it is known that the very old and especially the very young may contract the disease after ingesting (eating) only a few of these organism. Therefore, any food, especially a food that can be eaten without cooking, should be kept essentially free of these bacteria.

The bulk of human salmonellosis is clearly related directly to man’s association with animals, both wild and domestics. Salmonella are host adapted; however most Salmonella are not host adapted. Although any Salmonella is a potential pathogen for humans, most food borne salmonellosis is caused by non-host adapted serotypes.

Improper cooling, inadequate cooking of thermal processing, ingestion of contaminated raw products and cross contamination were frequently identified as contributing factors in the implicated foods. Raw foods of animal origin are the major sources of salmonellae in kitchens of restaurants, institutions and homes. Salmonella may survive in these foods as a result of improper cooking, more commonly, cross contamination of foods after cooking is the source of Salmonella. Foodservice workers or in home preparers may transfer salmonellae from raw products to cooked or other uncontaminated foods as a result of not washing between handling of these foods. Salmonella can also be transferred from contaminated raw foods to equipment surfaces e.g., knives cutting boards, counter tops, etc, and then from equipment to previous uncontaminated foods. Once contamination occurs, the situation may further complicated by insufficient storage of the product before serving. Contaminated foods are sometimes kept at room temperature, improperly refrigerated, or held in warmers within the growth range for salmonellae.

Although responsible for fewer outbreaks, contamination of foods by infected workers cannot be ignored as a cause of foodborne salmonellosis. Improper hygiene practices by these individuals may lead to either contamination of foods or direct person-to-person contamination. Historically egg-products were a significant source of human salmonellosis in the United States but mandatory pasteurization of egg products has been responsible for greatly reducing eggs as an important cause of salmonellosis.
