Epidemiology of Shigellosis

Shigellosis, an intestinal infection caused by Shigella bacteria, remains a significant public health problem worldwide, particularly in regions where clean water and adequate sanitation are limited. In many developing countries, shigellosis is endemic and contributes substantially to the overall burden of diarrheal disease. Studies estimate that Shigella accounts for 10–20% of all enteric infections in these settings and is responsible for nearly half of all cases of bloody diarrhea, or dysentery, among young children. The disease is especially common in children under five, whose still-developing immune systems make them more vulnerable to infection. After this age, the prevalence drops sharply as immunity builds and exposure patterns change.

In contrast, developed countries experience shigellosis in different epidemiological patterns. While the overall incidence is lower due to better hygiene and sanitation infrastructure, outbreaks still occur sporadically. These typically stem from a single contaminated food or water source, such as improperly handled produce, recreational water exposures, or food prepared under poor sanitary conditions. Because Shigella spreads easily through the fecal–oral route, even small lapses in hygiene can trigger localized outbreaks.

Certain environments in high-income countries can sustain pockets of endemic transmission. Child-care centers, for example, are particularly vulnerable due to close contact among young children and the challenges of maintaining strict hygiene practices. Similar risks are seen in institutional settings such as nursing homes, correctional facilities, and shelters, where crowding and shared amenities can facilitate rapid spread. Remote or marginalized communities with inadequate sanitation infrastructure may also face a higher risk, mirroring conditions more commonly seen in developing regions.

Globally, shigellosis remains a major cause of illness and death among children, although mortality has declined in recent decades due to improved access to treatment and better nutrition. However, the rise of multidrug-resistant Shigella strains poses a growing challenge, complicating treatment and increasing the potential for persistent transmission. Continued investment in sanitation, safe water access, hygiene education, and surveillance is essential to reducing the global burden of this highly contagious disease.

Epidemiology of salmonellosis

On a global scale, Salmonella is responsible for an estimated 3 billion human infection annually. The salmonellae are gram-negative, non-spore forming, facultatively anaerobic bacteria in the family Enterobacteriaceae.

Salmonellosis is an illness caused by Salmonella bacteria. An infection with these bacteria usually affects the gastrointestinal system (the stomach and intestines) in humans. In more severe cases, Salmonella can spread to the blood, the bones, or even to the fluid around the brain, but these types of infection are less common.

There are numerous transmission pathways through which humans can be exposed to Salmonella, including a wide range of domestic and wild animals and a variety of foodstuff’s including food of both animal and plant origin.

Infected animals carry Salmonella in their feces and the usual route of infection is through fecal-oral transmission. The epidemiology of Salmonella is, therefore, primarily due to direct or indirect fecal contamination of live animals, food or humans.

Infections caused by the salmonellae are group into three major syndromes: gastroenteritis, typhoid or enteric fever and localized infection outside of the GI tract.

In developed countries, most cases of Salmonella infections are acquired upon ingestion of contaminated poultry and beef products.

Most cases of typhoid fever occur in the developing world, particular the Indian Subcontinent and South East Asia, but the disease is also endemic elsewhere in Asia and in Africa. The disease is also commonly reported from the Middle East.

Epidemiology and Foodborne Illnesses
Most of what is known about foodborne illness started with epidemiology, the study of disease in a population. John Snow, a London physician, used deductive reasoning, research and interviews in the 1880s to determine the cause of a cholera epidemic that had killed more than 500 people in one week.

Scientists used Snow’s technique to investigate primarily infectious diseases until the 1920s, when the field broadened to include clusters of all factors that apply to the incidence of disease among people.

Epidemiological techniques have improved over the years, In 1970s, Dr. Paul Blake developed the case control method.

This method compares those who became ill with closely matched individuals who stayed well.

By examining what those who became ill did differently from those who stayed well, the source of infection can often be revealed.

In the case of foodborne illness an ill person is question about where and what they ate and matched as closely as possible in age, health status and eating pattern to someone who stayed well in an effort to pinpoint differences.

In the Centers for Diseases Control and Prevention (CDC) works to help treat and prevent disease at the national level and has increased its scope to lent epidemiological assistance worldwide because of the overlap between the developed and less developed worlds.

The people who pick and pack fruits and vegetables in foreign countries that are imported the United States are handling the US food supply.

If foreign worker have illness that can be transmitted through food, their illness have a direct bearing on our health.

Foodborne illness is most often linked to bacteria, but there are other agents that can cause foodborne illness, including viruses, parasites, prions and molds.

Bacterial illness is the most prevalent but viruses and parasites are being spread through food more commonly than in the past.

Each types of disease agent has different characteristics that must be considered when implementing food safety strategies.
